Pursuant to the Freedom of Information Act, I hereby request the following records:

The Official Military Personnel File (OMPF) for Larry W Simmons including all Fitness Report and Counseling forms NAVPERS Form 1610/2, Enlisted performance evaluation reports NAVPERS Form 1616-1, the Background Summary (NAVPERS Form 1640/32), Most recent Cumulative Record of Training (NAVPERS Form 5300/16), history of assignments NAVPERS form 601-5 and/or NAVPERS form 1070/605, Navy Training History NAVPERS form 601-4, all military awards and full citations, military education/training courses and Form DD-214.
Also request decorations/ medal full Citations. Mr Simmons was a decorated Navy veteran of the Vietnam War.
If necessary please redact any sensitive information protected in accordance with the Privacy Act of 1974.
