Pursuant to the Freedom of Information Act, I hereby request the following records:
The Official Military Personnel File (OMPF) for JAMES F COOK including Enlisted performance evaluation reports NAVPERS Form 1616-1, the Background Summary (NAVPERS Form 1640/32), Most recent Cumulative Record of Training (NAVPERS Form 5300/16), navy occupational/training history NAVPERS 601-4 and Form DD-214.
Request history of assignments form NAVPERS 601-5 and NAVPERS Form 1070/605 and all military awards and citations.
Also specifically request Bronze Star Medal full citation and Navy Commendation medal full citation.
Mr Cook served in the United States Navy with service in South Vietnam during Vietnam War. If necessary please redact any sensitive information protected in accordance with the Privacy Act of 1974.
