Pursuant to the Freedom of Information Act, I hereby request the following records:

The Official Military Personnel File (OMPF) for CORT R TRAYLOR including Form DD-214, all Fitness Report and Counseling forms NAVPERS Form 1610/2, NAVPERS form 1610/1, Enlisted performance evaluation reports NAVPERS Form 1616-1, the Background Summary (NAVPERS Form 1640/32), Cumulative Record of Training (NAVPERS Form 5300/16), history of assignments NAVPERS 1070/605, Navy Training History NAVPERS form 601-4, all military decoration and full award citations, all military education/training courses, military service number.
Specifically request copy of history of assignments NAVPERS form 1070/605, copy of joint service commendation medal citations, joint service achievement medal citation, navy achievement medal citation, copy of navy unit commendation citation and copy of joint meritorious unit award citation with redactions as determined by FOIA officer.

Mr Cort R Traylor claims numerous decorations/awards including Vietnam Service medal, Republic of Vietnam Campaign Medal, Combat Action Ribbon, Armed Forces Expeditionary Medal, Armed Forces Reserve Medal, Humanitarian Service Medal and other decorations. Deployed to Southeast Asia during Vietnam War. Claimed former enlisted service member and navy commissioned officer, final rank O-6 Commander - US Naval Reserve. Approximate service dates 1970 to 2000 active duty and naval reserve duty. birth date unknown. This request is submitted to corroborate claim to be a U.S. military veteran and vietnam veteran. NPRC search unable to locate the OMPF file, advised to contact Navy Personnel Command.

PLEASE NOTE: I am general public and thus do not know the military service number.
I also request that, if appropriate, fees be waived as I believe this request is in the public interest . Claimed to be retired FBI Special Agent, government employee. The requested documents will be made available to the general public free of charge as part of the public information service at MuckRock.com, processed by a representative of the news media/press and is made in the process of news gathering.
