To Whom It May Concern:

This is a request under the Freedom of Information Act. I hereby request the following records:

All records that are related to or mention any program where an audit or customer service survey is used as a tool to close old FOIA requests without processing them. For example, FOIA request 20140067F was sent a letter on 10-27-14 that stated "this office is currently conducting an internal audit of our FOIA program to track our level of customer service satisfaction and response effort." Additionally, the letter stated "we hope that you received a satisfactory response to your request." However, upon follow up, I was told that your office knew that there had been no response to the request and you intended to close the request if the customer service survey/audit was not responded to. This new FOIA request is for all records discussing the use of an audit/survey to close FOIA requests without providing any records to the requesters including the directions to conduct this activity, the meeting minutes discussing the possibility of using the audit in this way, and all other records that mention or are related to the use of audits and customer service surveys. You can excluded communications between your office and individual requesters, but include all internal communications about the program in general.

I also request that, if appropriate, fees be waived as I believe this request is in the public interest. The requested documents will be made available to the general public free of charge as part of the public information service at MuckRock.com, processed by a representative of the news media/press and is made in the process of news gathering and not for commercial usage.

In the event that fees cannot be waived, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 20 business days, as the statute requires.

Sincerely,

Jason Smathers
