This is a request under the Freedom of Information Act. I hereby request the following records:

I request a flat file (Excel or CSV) output digital/electronic file of the dataset associated with the Consolidated Hotline System, which includes the Safety Hotline and the Administrators Hotline., which is administered by the FAA Office of Audit and Administration. I am not requesting a paper copy, but rather a digital copy on a CD or DVD.

Please limit the data to fiscal year 2012 only.

Please include all fields except those assigned to personal information such as name, address, phone number and email address. Those you may exclude. Please include the narrative field, if one exists.

I also request that, if appropriate, fees be waived as I believe this request is in the public interest. The requested documents will be made available to the general public free of charge as part of the public information service at MuckRock.com, processed by a representative of the news media/press and is made in the process of news gathering and not for commercial usage.

In the event that fees cannot be waived, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 20 business days, as the statute requires.

Good Afternoon Mr. Morisy,

Per your FOIA request the estimated cost for the requested information
from the Safety Hotline for 2 hours would be $250. This does not include
the review time, which means the total cost will be higher. Please note
the Safety Hotline and Administrator's Hotline are 2 separate databases.
The Office of Audit and Evaluation is still waiting for the cost for the
data from the Administrator's Hotline. Please let me know if you would
like for our office to proceed forth with your request.
