This is a request under the Freedom of Information Act. I hereby request the following records:
Copies of all correspondence, memorandum, meeting agendas, minutes, and any other documents, made by the DoD Pharmacy & Therapeutics Committee or members thereof acting in their official capacity as members of the committee, related to planned, proposed, or enacted policy for the use of tafenoquine (Arakoda) by the U.S. military, to include any records related to the potential inclusion of tafenoquine on the DoD formulary, as discussed in the Tafenoquine Transition Planning Meeting (enclosed).
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
