Pursuant to the Freedom of Information Act, I hereby request the following records:

All correspondence, including but not limited to agreements, contracts, correspondence, documents, emails, memoranda, and/or internal communications related to personal protective equipment (PPE) such as masks between the Indian Health Service and Zach Fuentes, Sia N. Ashok, Zach Fuentes LLC, or any similar forms of these names. This should include any post-sale correspondence, including but not limited to correspondence with White House personnel regarding the contract or Zach Fuentes, quality concerns, complaints, testing, recourse, refunds, and/or additional orders.

A reasonable search would begin January 1 2020 and continue until the date this FOIA is fulfilled.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 20 business days, as the statute requires.

As your status request does not appear to be a complete representation of the communications with IHS on case 20-116, please advise whether or not you have received communication from Acting IHS FOIA Officer, Evonne Bennett. Our records indicate it was successfully sent on 11/23/20.

Also, as previously stated, IHS has does not provide completion date estimates given the pandemic crisis and the significant current backlog.
