To Rachelle Lawendowski:

On behalf of the Freedom of Information Act Officer of the Office of Intelligence and Analysis in the Department of Homeland Security, I am responding to your email request, reproduced below. Your follow-up correspondence was the first notice we had that you had submitted this request.

Your request for information concerning yourself is processed under both the FOIA and the PA to afford you maximum access to records. When submitting a request for copies of records about yourself, we must verify your identity to ensure personal information is not released to anyone other than yourself. The DHS regulations, 6 CFR Part 5 § 5.21(d), require verification of your identity, including your full name, current address and date and place of birth. In addition, your request must be made in writing, must contain your signature, and should either be notarized or contain a statement made under penalty of perjury as permitted by 28 U.S.C. 1746.

Although you have provided your full name and date of birth, we need additional identifying details. Please resubmit your request with the information requested above. A scanned copy of a notarized signature or a statement made under penalty of perjury is acceptable. If we do not hear from you within 30 days from the date of this message, we will assume you are no longer interested in this FOIA/PA request, and the case will be administratively closed. Please be advised that you will be considered an "all other" requester for purposes of determining fees. When contacting this office again about your request, please refer to 14-OIA-0016.

If you construe this action to be a denial of access, you have a right to appeal it administratively within 60 days of the date of this letter to: Associate General Counsel (General Law), U.S. Department of Homeland Security, Washington, D.C. 20528, following the procedures outlined in the DHS regulations at 6 C.F.R. § 5.9. Your envelope and letter should be marked "FOIA Appeal." Copies of the FOIA and DHS regulations are available at www.dhs.gov/foia.
