This is a request under the Freedom of Information Act. I hereby request the following records:

1. The most current 14C filings submitted by all organizations in the state of New York. The 14C document is titled, “Application for Authority to Employ Workers with Disabilities at Special Minimum Wages," and should include supplemental sheets listing worker pay, disability, work address and more.

2. The Department of Labor's record retention schedule in digital format, if possible.

I am willing to accept partial responses as they are gathered, instead of waiting for all documents to be compiled.

This material will be made available to the general public. I am requesting a waiver of all fees under 5 U.S.C. Section 552(a)(4)(A)(iii) due to the fact that this is a public interest story -- the majority of which concerns governmental operations (sheltered workshop funding sources, oversight and government agency involvement) and is not in my commercial interest.

We understand that worker names need to be redacted.

If you do not see fit to grant a waiver of fees, please advise me of the cost prior to compiling any documents, along with a breakdown of those costs.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that fees cannot be waived, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 20 business days, as the statute requires.
