This is a request under the Freedom of Information Act. I hereby request the following records:

I am seeking documents that the Department of Defense has produced or gathered on the topic of dietary supplements from January 1st 2010 through Thursday, June 30th, 2016.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes. I am a member of the media and I request that any fees be waived.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 20 business days, as the statute requires.

Sir,

In regards to your FOIA request reference above number and your other FOIA number 26505-90115671 I will need some clarification.

Are you asking for specifically Fort Campbell, Kentucky or do you want all of Department of Defense? Also, they look to be asking for the same thing except the first one (26505) is a little more detailed. You might want to consolidate and make one request instead of two.

If you are wanting just Fort Campbell, Kentucky information then this would need to go thru the Medical Command asking for information from the Blanchfield Army Community Hospital. A request needs to be sent to that agency. Their address is:
