This is a request under the Freedom of Information Act. I hereby request the following records:

Any and all "DD Form 1532" reports filed for the Guantanamo Bay facility during the 2013 calendar year. The "DD Form 1532" reports detail the use of pesticides and other means of animal, pest and noxious plant control at DoD facilities.

The instructions on the form state that the "worksheets were formatted as an excel spreadsheet so that they can be easy for the user to complete and compatible for permanent archiving by the US Army Public Health Command." As such, these records should be easily located and easily provided in their original digital format.

Department of Defense Instruction 4150.07 states that DD Form 1532 must be filed monthly for "all DoD operations, activities, and installations worldwide.”

I also request that fees be waved as I am a member of the news media and intend to use any documents to educate the public.

I also request that, if appropriate, fees be waived as I believe this request is in the public interest. The requested documents will be made available to the general public free of charge as part of the public information service at MuckRock.com, processed by a representative of the news media/press and is made in the process of news gathering and not for commercial usage.

In the event that fees cannot be waived, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
