FEE WAIVER REQUEST: The requested records will be published and made available to the general public, and this request is not being made for commercial purposes. The requested records will be used for non-commercial research and journalism purposes, and will significantly contribute to an understanding of government activities by the general public. In light of recent safety concerns about these devices, including falls and fires, publication of the data will benefit the public through enhanced consumer safety and awareness.

In the event that fees cannot be waived, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
