Pursuant to the Freedom of Information Act, I hereby request the following records:
Records of any type, from the time period beginning January 2017 through the present, pertaining to the purchase of, provision of, use of, or communication regarding:
1. Tanning Equipment (For instance, but not limited to: spray tanning booths or UV tanning beds)
2. Tanning supplies (For instance, but not limited to: any chemicals or additional items needed to operate tanning equipment: sunless tanner chemicals, tanning bed lights, personal tanning supplies like eye/nose/mouth covers)
3. Tanning services (For instance, but not limited to: payments made to any company or individual for the purpose of: obtaining a tan, operating tanning equipment, or using tanning supplies )
4. Transportation to and/or from any location that provided tanning services.
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request.
I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
Thank you in advance for your anticipated cooperation in this matter.
I look forward to receiving your response to this request within 20 business days, as the statute requires.
