This is a request under the Freedom of Information Act. I hereby request the following records:
A copy of the document described as a "confidential mefloquine report" sent as an email attachment by FDA to Dr. Paul Arguin (pma0@cdc.gov) at the Centers for Disease Control (CDC) on or about August 8, 2013 (see responsive documents at https://www.muckrock.com/foi/united-states-of-america-10/malaria-division-documents-related-to-the-mefloquine-boxed-warning-15907/). In processing this request, please be advised the report may have a filename similar to "rev_cases of disabling neuropsych events mefloquine 5 24 2013 eos ja (2)addendum_Redated.pdf", and may have been sent by Dr. Sumathi Nambiar (sumathi.nambiar@fda.hhs.gov) at FDA CDER.
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that fees cannot be waived, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
