COMPLAINTS WRITTEN ON ME BY COWORKERS (Bureau Of Indian Affairs - Districe V Office - Rocky Mountain Region)

To Whom It May Concern:

Pursuant to the Freedom of Information Act, I hereby request the following records:

I am requesting to obtain any and all complaints submitted on me (Suzette King-Capitan) by my coworkers to include any witness statements or any material evidence. I am requesting full details of the any and all complaints. I am entitled to see all parts of any complaints as they relate to me. Subsequently, I am also requesting the result of complaints i.e. who they were forwarded to, who read them, copies of any meeting minutes that have take place and the recommendations or result of the complaints.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 20 business days, as the statute requires.
