Pursuant to the Freedom of Information Act, I hereby request the following records:
According to the administrator of Colonial Manor of Amana, IA., the skilled-nursing facility has been the subject of "3 infection control surveys conducted by state and CMS surveyors since March 2020" related to COVID-19.

I am requesting those inspection records be released.
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 20 business days, as the statute requires.

This is a public records request like any other. I am not looking for a quote or typical media materials. I am looking for the release of the records I've requested under the Freedom of Information Act.
How your office handles that internally is up to you. You are already beyond the 20 business day deadline you outlined in your acknowledge letter. Please provide a response to my records. There should be no reason I have to fill out a separate form or begin my response again. There is nothing in FOIA that says you can slow down my request or start it over based on my status as a member of the media. That is discrimination.

We have completed our review and determined that we are not authorized to release the documents to you in full because they contain protected health identifiers. Please find 8 pages of the CMS-2567, a copy of your CMS correspondence letter, Acknowledgement letter & a copy of your FOIA request.
