Center for Medical Progress 501 c3 Application

To Whom It May Concern:

This is a request under the Freedom of Information Act. I hereby request the following records:

I request any and all forms related to the Center for Medical Progress' (CMP) application for status as a charitable organization. The CMP was founded in 2013 by David Daleiden and has appeared in the news recently for its connection to secret recordings of Planned Parenthood meetings. These forms should include but not be limited to:

-initial registration forms (Form 1023 or 1023-EZ)
-bylaws
-Form 990, Form 990-EZ, 990-N or Form 990-PF
-FORM SS-4

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that fees cannot be waived, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
