CDL CLASS A RECORDS AND CLASS C DRIVING RECORD FOR PAST 3 YEARS

To Whom It May Concern:

Pursuant to the Freedom of Information Act, I hereby request the following records:

Persuant to the Freedom of information act, ratified July 5, 1967, I am requesting all records and information that is available to this department, pertaining to my driving history for the past 3 years. This may include, but, is not limited to: moving violations, tickets, citations, expirations, accidents etc. In addition, I am requesting any and all records, history and all other forms of information pertaining to any commercial driving for the past 3 years, that is available to this department. Once more, this may include, but will not be limited to: all past employment history as well as driving records such as violations, accidents, citations and so on. The documents may be E-mailed to:
