To Whom It May Concern:

Pursuant to the Freedom of Information Act, I hereby request the following records:

1) Copies of all monthly activity reports and evaluation forms, as described in the attached memorandum of understanding, completed by employees of the New York City Police Department participating in missions abroad through the Bureau of International Narcotics and Law Enforcement Affairs (INL) since Jan. 1, 2009. Please note that the memorandum of understanding is attached as evidence of the existence of these records and an aid in finding them. I am not limiting my request to just the 2013 mission to Haiti, I am requesting all monthly activity reports and evaluation forms prepared by employees of the NYPD during any deployment abroad in participation with INL during the specified time period.

The MOU states on page 4:

"Once in Haiti, the team will report to the NAS Director through the NAS Senior Police Advisor. One NYPD Technical Advisor shall be designated the 'lead officer' who will be the direct liaison officer to the NAS. Each NYPD Technical Advisor will also regularly report to the New York City-based program accountability officer. INL will require monthly reports from the NYPD team on the ground detailing their activities, recommendations, and progress."

And the MOU states on page 9:

"Furthermore, officers will be evaluated by their lead officer on their performance throughout their deployment period. The lead officer will prepare an evaluation report highlighting the officers' accomplishments and based on the members performance will recommend extending the members for a second deployment or redeployment at a later time. The final decision to extend a member will rest with NYPD."

Ii ask that all fees be waived as I am a journalist and I intend to use the requested records to publish articles in the public interest and not for any commercial purpose.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 20 business days, as the statute requires.
