Pursuant to the Freedom of Information Act, I hereby request the following records:
A copy of all address certificates.
A copy of all relevant information from each address certificates in .csv format.
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 20 business days, as the statute requires.

This information would be obtained from the City of Chicago Department of Transportation. We are part of the Federal government and would have nothing to do with house number address certificates for the City of Chicago. Because the records you seek are not maintained by the Federal Aviation Administration or the federal government Department of Transportation, this request will be closed with no further action being taken by the agency. Please contact the City of Chicago Department of Transportation for the requested records.
