This is a request under the Freedom of Information Act. I hereby request the following records:

All records of religious accommodation requests submitted to the agency under the ACA’s contraceptive mandate, as well as other ACA contraceptive mandate exemptions submitted that are not based on religious exemptions.

I request the communications submitted to the agency requesting ACA contraceptive mandate exemptions, as well as any databases, spreadsheets, or other logs of such exemption requests and the current status of the requests.

I also request any materials created to evaluate such requests, as well as any prior versions of guidance used to evaluate such requests. For this part of the request, please include legal memos, guidance, PowerPoint and other presentation materials, electronic training, and other guidance provided to individuals who evaluate exemption requests in accordance with both new guidance
provided by the executive branch (as referenced here https://www.nytimes.com/2017/10/06/us/politics/trump-contraception-birth-control.html ) as well as processes to evaluate contraceptive mandate exemptions.

For help processing this request, I note that similar information was previously released after a FOIA request filed by American Progress; my request includes both the data released in response to that request as well as data generated after the date that their request was processed.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
