Pursuant to the Arizona Public Records, I hereby request the following records:

I am requesting health inspection, safety and food-borne illness at the following locations in Tucson Arizona.
From August 2017 to March 2018 from Trinity Presbyterian Church, Holy family Catholic Church, Tucson Indian Center(food box sites)
Community food bank of Southern Arizona 3003 S Country Club Rd, Tucson, AZ 85713
Homeless shelter: Sister Jose Women's Shelter

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Thank you in advance for your anticipated cooperation in this matter. I would request your response within ten (10) business days.

Department, and therefore all records of inspections and results would
fall under their jurisdiction, not the City of Tucson.
