Pursuant to the Texas Public Information Act, I hereby request the following records:

Seeking a "Marriage Verification" document to be sent via US Mail, regarding 1974 marriage in Texas of:
current presidential candidate Jeb Bush (John Ellis Bush) and Columba Bush if indeed such document is a public record, and request that it be certified if possible. The requested document is described here on your website:

http://www.dshs.state.tx.us/vs/reqproc/faq/mardiv.shtm

Can you tell me the date and place where someone was married or divorced?
Verification letters of marriage are available for marriages that occurred from 1966 to present.
----------------------

My understanding is that a certain form is required to be filled out and sent to your office in addition to this request (Form VS-142.9) and the document search costs $20 (twenty dollars) whether you find the document or not:

http://www.dshs.state.tx.us/vs/reqproc/faq/mardiv.shtm

Download the VS-142.9- Mail Application for a Verification Letter (PDF) (48K). The fee to search our files and indexes is $20. The fee is retained as payment for the search, even if a record is not found.
-------------

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that fees cannot be waived, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 10 business days, as the statute requires.

Sincerely,

Susan Alyn

You can go to our website at: http://www.dshs.state.tx.us/vs/reqproc/forms.shtm to obtain an application form. Or, you can search our marriage indexes by going to our website at: http://www.dshs.state.tx.us/vs/marriagedivorce/mindex.shtm.

Please let me know if you need any further assistance.

From: Susan A.

I'm not sure what you're asking for or why there is "fix" required -- your document claims someone requested a birth record but no one did.

The $20 was paid, and the only form you required does not ask for a photo ID as now mentioned in your "fix."

Why are you now asking for photo ID in your "fix"?

I think you made some mistakes, and no "fix" is actually required or needed.

Please clarify what you are doing and why with your "fix" letter.

Terri, hello-
Can you please identify the Texas Open Records Act statute that requires that the requester provide photo ID? Thank you.

From: Hendricks,Terri (DSHS)

Good morning Susan,

The statute regarding identification falls under Rule 181.28, i9.

I'm sorry, but I don't see anywhere on this URL:
http://www.dshs.state.tx.us/vs/reqproc/faq/mardiv.shtm
or on the form one is required to download, any statute
or request that a person provide photo ID to the State of Texas in addition to the form and the $20.

Where is this request for photo ID publicly shown in the information publicly provided above?

From: Susan A.

Terri Hendricks' letter to MuckRock dated Dec 16, 2015, appearing publicly on this thread, erroneously claims the request is for a birth or death certificate. However -- that is not the document request being made here.

Please reread the request, for which $20 was already paid to you, as the requested document is for the following document:
"Marriage Verification" document.

And nowhere on that document form does it require a photo ID to obtain.

Thank you.

From: Susan A.

PS Here again is the URL on your site for a "Marriage Verification" form:

http://www.dshs.state.tx.us/vs/reqproc/faq/mardiv.shtm

Nowhere on that page or on the application form is there a requirement stated that a photo ID is required.

MuckRock has already sent you the $20. Please send the requested "Marriage Verification" document or state you don't have one for John Ellis Bush (Jeb Bush). Thank you.
