Pursuant to the Texas Public Information Act, I hereby request the following records:

Any and all records relating to records and reporting, in the ImmTrac2 system, including, but not limited to, the number of doses of COVID-19 vaccine and adjuvants that were used, unused, spoiled, expired, or wasted by registered COVID‑19 vaccine providers in Texas (https://www.dshs.state.tx.us/coronavirus/immunize/provider-information.aspx). These records and reporting are referenced by the Texas Medical Association (https://www.texmed.org/TexasMedicineDetail.aspx?id=55450&utm_source=Informz&utm_medium=Email&utm_campaign=TMT&_zs=eJsdA1&_zl=TkK66). We are requesting all records generated since November, 2020.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

(g) The department may release nonidentifying summary statistics related to the registry that do not individually identify an individual.

We do have a report posted on our website that is updated weekly that shows summary data on doses wasted as reported by vaccine providers in ImmTrac2. You can find that report here: https://www.dshs.texas.gov/news/updates.shtm#waste

Doses used and unused are not tracked by providers in ImmTrac2.
