for Village Behavioral Health, from 01/01/14 until the present.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

In order to process the request for records regarding Village Behavioral Health, I need to receive proof of Tennessee citizenship from the requester. That proof can be a scanned image or photograph of a valid Tennessee driver’s license or Tennessee Identification Card. Upon receiving proof of citizenship, I will move the request forward gathering respondent documents and working to estimate a cost (if applicable) for copies and redaction.
