Pursuant to the Tennessee Public Records Act, I hereby request the following records:
I seek copies of the winning proposal and awarded contract for the Health Management and Wellness Services for the State's Public Sector Plans solicitation. The contract was awarded on January 1, 2011. The contact person for the solicitation was Marlene D. Alvarez, Procurement and Contracting, Tennessee Department of Finance & Administration. Ms. Alvarez may be reached at marlene.alvarez@tn.gov or 615.253.8358.
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
