Pursuant to the Tennessee Public Records Act, I hereby request the following records:
I seek information regarding the RFP for Health Care Finance and Administration Member Service Center (RFP # 31865-00380). The contract was awarded sometime in 2015. The contact person for this RFP was Alma Chilton. Ms. Chilton can be reached at alma.chilton@tn.gov. Specifically, I seek the following: all submitted proposals (technical and cost); orals presentation documents, BAFO documents and score sheets for all vendors; monthly, quarterly, and annual performance reports for the contract period (August 31, 2018 – Aug 31, 2020); and monthly, quarterly, and annual invoices for the contract period (August 31, 2018 – Aug 31, 2020).
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
