Somerville Housing Authority Project-Based Voucher Sites

Pursuant to the state Freedom of Information Act, G.L.M. c.66, §10, I hereby request the following records:

Please provide:
1. a complete list of all SHA-maintained PBV sites;

2. the complete and most current SHA-maintained PBV site-specific waiting lists, substituting an ID number (#) for each applicant's name, and with the following information intact:
a. date/time stamp for each application;
b. preference and eligibility selection factors named for each applicant; and
c. family composition for each applicant; and

3. SHA's Identification of Housing Needs statement or report, that was submitted with SHA"s most recent Five-year Plan.

SInce this information is in the public interest, and requested in order to provide community participatory comments regarding SHA's 2012 Annual Plan draft if necessary and appropriate, we expect that there will be no charge for this public information.

In the event that fees cannot be waived, I would be grateful if you would inform me of the total charges in advance of fulfilling my request ASAP.

Please fill this request by accessible (not scanned-image .pdfs) electronic format, by e-mail attachment if available or CD-ROM if not.
