Pursuant to the state Freedom of Information Act, G.L.M. c.66, ยง10, we hereby request the following records:

1. The SHA 2012 Annual Plan in accessible (not scanned-image .pdf) format.

ALTERNATIVELY: Please substitute the current inaccessible-format SHA online version -
which is at:

with an accessible document;
and then, please send the url for that replacement document in your response to this request.

2. Please send a complete listing of all physically accessible (per 24 CFR 983.192) PBV sites in Somerville, MA (including SHA-owned units), providing this info:
a. exact location address and
b. total PBV units by size(s).

ALTERNATIVELY: If this updated information is currently online; and, already in accessible format: please send the link for that information in your response to this request.

SInce this information is in the public interest, and requested in order to provide community participatory comments regarding SHA's 2013 Annual Plan draft if necessary and appropriate, we expect that there will be no charge for this public information.

In the event that fees cannot be waived, we would be grateful if you would inform us of the total charges in advance of fulfilling this request.
