Pursuant to the Massachusetts Public Records Law, M.G.L. c.66, ยง10, I hereby request the following records:

Please provide
1. documentation showing when and who (persons or company) completed the most recent ADA/Section 504 Self-Evaluation for the Somerville Public Schools facility and programs located at

42 Cross Street
Somerville, Massachusetts 02145-3246

2. Please provide a copy of that ADA/Section 504 Self Evaluation report on this facility.

3. In the event that the ADA/Section 504 Evaluation on this facility was completed prior to March of 2012, then please provide dated documentation showing that an ADA/Section 504 TRANSITION PLAN for the Somerville programs located at 42 Cross St. has been written AND completed; OR, is in the process of being completed.

We request that, if appropriate, fees be waived as suggested but not stipulated by the recommendations of the Massachusetts Supervisor of Public Records, since this request is made in the public interest for purposes of information gathering relative to citizen participation in Federally-funded and other-funded programs and activities. The requested documents will be made available to the general public free of charge as part of the public information service at MuckRock.com, processed by a representative of the news media/press and is made in the process of news gathering and not for commercial usage.

In the event that fees cannot be waived, please inform us of the total charges in advance of fulfilling this request.

Per Federal and State laws, regulations and/or guidelines, this public information request must be filled in an accessible format, so that it can be read by persons using machine- reader technology. Please note that scanned-image .pdf files are image files and therefore are not accessible to machine-readers; nor are printed documents. In the event that the records sought are not computer-generated documents, then of course we accept printed copies or electronic image files.
