Pursuant to the California Public Records Act, I hereby request the following records:

Please provide all general complaints, guest requests, life safety transfer requests, reviews of requests, request logs, placement opportunities, reasonable accommodation requests, and requests for readmission, appeals, decisions, and post-appeals for all SIP hotels and Safe Sleep sites. For examples of descriptions of such complaints and requests, see the pages 47 through 54 of this SIP Hotel Operations Manual and Guidance document: https://sfgov.org/sheltermonitoring/sites/default/files/SIP%20Hotel%20-%20Operations%20Manual%20%26%20Guidance%20Updated%20201216.pdf

Please include the results or consequences of each request or complaint in the categories above along with the requests and complaints themselves.

If data exists on number of complaints/requests, % of complaints/requests resolved, status of resolution, etc. please provide that data, name the database it originates from, and provide its data dictionary.

Where relevant, please note whether the complaint or request involves any individual or service from Urban Alchemy or is related to Urban Alchemy in any other way.

Please provide results on a rolling basis in a searchable format. If possible please release complaints first and move on to the remaining items afterwards.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

I apologize for the delay. Thank you for your patience as we identified responsive documents.

We have attached an initial batch of responsive documents. HSH staff is continuing to locate and review additional responsive non-privileged documents and will provide those documents to you on a rolling basis. S.F. Admin. Code § 67.25 (d).

Personal information included in responsive documents, but not subject to disclosure under S.F. Adm. Code Sec. 67.24(c) and personal cell phone numbers were redacted under Cal. Govt. Code Secs. 6254(c), 6254(a), and Cal. Const. Art. 1, Sec. 1.

Your request does not have a timeframe. Please note: HSH took over management of SIP and Safe Sleep logs in July of 2021. Prior to July of 2021 they were managed by the COVID Command Center, and prior to that, the Emergency Operations Center. We can continue to search for documents prior to July of 2021. However, if your interest in this subject is limited to HSH’s management of this data, we can limit your request to July of 2021 onward. Please advise.

Please continue to release the documents you know you have from July 2021 onward. After that if you find documents from before July that HSH doesn’t manage please release those as well. Please also include any supporting documentation relevant to the sleep logs along with that month’s log, including the original request forms filled out by guests.

Dear Anonymous,

Apologies for the inaccessibility of the Box folder. You should be able to access the files we released and be able to do so without creating an account. I will need to research this issue. In the meantime, I have attached the responsive documents to this email. Please confirm receipt.

As for providing the original complaints you requested, we have multiple pathways for guests to submit complaints. In addition to a staff member or guest filling out a request form, the requests also arrive by phone and in person. Consequently, we do not have an original request or complaint form correlated to each entry in the Guest Services Logs. Depending on what you are trying to measure, you will have gaps in information. My suggestion is that you see what you get as you receive the logs and look for gaps on an as-needed basis. Additionally, the retrieval of these forms on an individual client basis, along with the redaction process, will take considerable time. Given the timeframe, it could take 6-8 months to produce these documents. In some instances the amount of personal information that will need to be redacted will render some documents mostly devoid of useful information.

Hi according to DEM (see attached) HSH oversaw this work during the COVID response period and DEM does not have SIP Hotel and Safe Sleep Site logs from any period. Can you please double-check and confirm that you have no logs from prior to July 2021?
