Pursuant to the California Public Records Act, I hereby request the following records:

All correspondence between the Centers for Medicare and Medicaid Services at the US Department of Health and Human Services and the City of San Francisco Department of Health related to Laguna Honda Hospital from 1/1/2020 to 5/16/2022. City officials: Please file this as an "Immediate Disclosure Request" under San Francisco Administrative Code Section 67.25(a).

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Requester,

Please be advised that we are hereby invoking an extension of not more than 14 days from today, June 6, to respond to your request pursuant to the California Public Records Act. Under the Public Records Act, the deadline can be extended for up to 14 days due to “the need to search for, collect and appropriately examine a voluminous amount of separate and distinct records which are demanded in a single request” and “the need for consultation, which shall be conducted with all practicable speed, with another agency having substantial interest in the determination of the request or among two or more components of the agency having substantial subject matter interest therein.” (See Cal. Gov’t Code § 6253(c)(2) and (3)). 

DPH is invoking the extension on both grounds because there are a voluminous amount of records DPH staff must search through and examine to determine if it has responsive records, and DPH staff will need to consult with another City office regarding your request. DPH will endeavor to respond to you as quickly as possible, and no later than Tuesday, June 21.
