HE WAS SEEN AT "SMOKE BOMB HILL DENTAL MEDICAL CLINIC", WOMACK ARMY MEDICAL CENTER", AND "CLARK HEALTH CLINIC." HE WAS PRESCRIBED MEDICATIONS FOR INJURIES AND AILMENTS. I AM NEEDING ANY MEDICAL RECORDS AND PRESCRIPTION RECORDS DURING HIS ACTIVE DUTY PERIOD.
