In order to properly proceed with a request, our office needs an SF 180 to be filled out.
We have been unable to process the previous request due to lack of information i.e.
SSN, DOB, return Mailing Address, etc...
I have attached a blank copy of the SF 180 form that has all of the required information that needs to be submitted.
Once completed, please fax form over to 210-201-8310.
