Rhode Island Medicaid provider list

To Whom It May Concern:

Pursuant to the Rhode Island Access to Public Records Act, I hereby request the following records:

I am requesting a Rhode Island Medicaid healthcare provider list in electronic format (CSV preferred) that would include the providers NPI, Medicaid ID, and state license information. Specific items to be included in data set:
1. Provider name
2. National Provider Identifier (NPI) number
3. RI State Healthcare License number and initial date of issuance or expiry (if available/applicable)
4. Education / School/Training, if available
5. Provider type (MD, DO, PA, RN, PT, OT, OD, CRNA, DMD, DDS, DPM etc)
6. Specialty
7. Practice Location
8. Please include Data Dictionary describing state Medicaid Data Set
9. Please provide (if there is an existing) a static website link to verify an individual’s state healthcare licensure status and Medicaid status (accepting, not accepting new Medicaid patients etc).
If the costs of obtaining this data set exceeds $50 please inform me. I would like to request few waivers if there are significant costs, as this is performed for healthcare access and more transparent access to health care.
