Request for Contracts

To Whom It May Concern:

Pursuant to the Arkansas Freedom of Information Act, I hereby request the following records:

For context, this request is in regards to contracted healthcare providers your agency works with, including those who provide routine and emergency services to inmates.

I request the following documents created during the procurement, review, and renewal of health services:

Any Requests for Proposals (RFPs) issued by your agency, done on behalf of your agency, or done in coordination with your agency for healthcare services.
A copy of any proposals or bids submitted in response to the RFP or other bidding or proposal processes.
A copy of any sole source or no-bid justification documents generated for healthcare service provider contracts
A copy of any signed contracts for healthcare services, including any contract terms and riders, as well as any contractual terms that detail co-payment information, outsourcing costs, pregnancy, pre-natal, birth, and abortion costs.
Any post-award amendments or extensions of healthcare contracts.

Please provide documents that fall under the above criteria that were generated since Jan. 1, 2015 through the date this request is processed.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
