Pursuant to the Rhode Island Access to Public Records Act ("APRA"), I hereby request the following records:

(1) Procedures, Policies, Guidelines and Grievance Process for Parents to obtain a complete release of their child's records, to include all records, files, documents, notes, or other materials (electronic or otherwise), including but not limited to, any Cumulative file, Confidential file, Compliance File, or any other files maintained or acquired by the Portsmouth School Department, in addition to any records relating to decisions regarding said child's identification, evaluations, medical summaries, and medical notes, as required by the Family Educational Rights and Privacy Act, the Individuals with Disabilities Education Act, the Rehabilitative Act of 1973 - Section 504, the Americans with Disabilities Act, and/or by any other federal or state statute.

This is to include all records referring to said child in any personally identifiable manner, i.e. records containing said child's name, Social Security number, Student ID number, or other data making them traceable to said child.

(2) Procedures, Policies, Guidelines and Grievance Process for Parents to correct the inaccuracies in said records, as required by the Family Educational Rights and Privacy Act, the Individuals with Disabilities Education Act, the Rehabilitative Act of 1973 - Section 504, the Americans with Disabilities Act, and/or by any other federal or state statute.

The requested documents will be made available to the general public free of charge as part of the public information service at MuckRock.com, and is not being made for commercial usage.

In the event that fees cannot be waived, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 10 business days, as the statute requires.
