Pursuant to Pennsylvania Right to Know Act, I hereby request the following records:
Time period of request: 04/01/2015 - 04/01/2016
Copies of all computer equipment (computers, laptops, cell phones, tablets) removed from PPS possession. Including lease returns, sale of units, recycling of units or donation of units. Records include but are not limited to invoices, credit memos, checks, bills of lading, email correspondences, contracts, recycling certificates, data destruction reports and asset reports.

All data destruction records of hard drives, cell phones and other data containing electronic equipment.
Time period of request: 04/01/2015 - 04/01/2016
Due diligence performed on vendors who equipment was transferred to. Records including but not limited to E&O insurance, Crime insurance, pollution insurance, cyber insurance, PADEP permit, 3rd party certification including R2 or e-Stewards and conflict of interest affidavit.
Time period of request: 04/01/2015 - 04/01/2016
Copies of any notices, warnings or fines relating to environmental compliance received by Pittsburgh Public Schools.
Time period of request: 04/01/2015 - 04/01/2016
Copies of any data breaches, HIPAA violations or other information security notices, warnings or fines.

Copy of Pittsburgh Public Schools data breach policy or Standard Operating Procedure (SOP)

Copy of Pittsburgh Public Schools data destruction policy or Standard Operating Procedure (SOP)

Copy of Pittsburgh Public Schools data security policy or Standard Operating Procedure (SOP)

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
