Pursuant to Pennsylvania Right to Know Act, I hereby request the following records:

This is a request for any public records with regard to the (presumably) November incident involving "Joshua Cordova", video of the incident which
may help provide respondent records is located here ( and reported on here ( while it is impossible to determine
the precinct involved the initial police vehicle number 3526 was the primary vehicle on scene which may aid in identifying respondent records.

Specific records should include all public arrest and report records, as well as any body cameras and 911/non-emergency recordings regarding the incident.

If you have any questions please feel free to ask.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 5 business days, as the statute requires.

Hi, I believe that this is an incorrect rejection of my FOIA/RTK request and this is an appeal, While I /did in fact/ submit this request with a proper right to know form as MUCKROCK is want to do; I have filled out another right to know form and submitted it to you directly, as well as mailing a physical copy, and addressing this thread as an appeal to look at the form that was already submitted with the original request which clearly states "please use additional sheets as necessary", and also clearly has an email checkbox to allow me to submit my request in writing.. _by email_.. but just in case my direct email are scans of physcal paper and comply to the letter with "in writing" and it compliant with current philly RTK laws and policies.
