To Whom It May Concern:
Pursuant to Pennsylvania Right to Know Act, I hereby request the following records:
A copy of the Philadelphia Police Department's annual "Equitable Sharing Agreement and Certification" forms for the fiscal years 2009-2013. I originally submitted this request to the police department, which said the district attorney's office maintains these forms. The forms must be submitted each year to the DOJ in order for the police department to participate in the federal equitable sharing program.
Attached, please find an official Right To Know form for this request.
I also request that all fees be waived as I am a journalist -- employed by MuckRock and the Boston Globe -- and intend to use the requested documents to educate the public on the DOJ's equitable sharing program.
I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 5 business days, as the statute requires.
