Pursuant to the Pennsylvania Right to Know Act, I hereby request the following records:
All Pennsylvania State Athletic Commission records pertaining to MMA fighter Levan Makashvili. This includes all materials for his license to fight (application, released medical records, etc.), all materials related to his two matches held in Pennsylvania (e.g., results, forms concerning broadcast, ticket accounting records, etc.), as well as the licenses for the Seconds, Cornermen, Manager and Promoters of record for his two matches.
I also request any associated correspondence the PSAC holds regarding Makashvili's ongoing right to fight in Pennsylvania, including materials pertaining to any investigation or suspension.
To identify fight records, these two fights held in Pennsylvania are listed below with date, opponent, event name and venue:
February 22, 2014, against Jordan Stiner, at CFFC 32 at the Valley Forge Casino Resort in King of Prussia.
December 13, 2014 , against Alexandre Bezerra, at CFFC 44 at the Sands Casino Resort in Bethlehem, Pennsylvania.
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 5 business days, as the statute requires.
