Pursuant to the Pennsylvania Right to Know Act, I hereby request the following records:
1) All records including contracts, estimates and email correspondence with Advanced Recovery, Inc (time frame: 01/01/2018 - 04/15/2018)
2) All due diligence record on Advanced Recovery, Inc. Including copies of R2 or E-Steward certifications, copies of insurance, and state permits.
3) Copy of DEP form 2500-FM-BWM0612 for event dated 4/14/2018
4) Copy of contract or agreement with Penn State Great Valley, in relation to the event on 4/14/2018
5)Copies of all correspondences with Penn State Great Valley in relation to the event on 4/14/2018
5)Copies of all correspondences with PA DEP in relation to the event on 4/14/2018
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
