Pursuant to Pennsylvania Right to Know Act, I hereby request the following records:

I hereby request that you produce the requested information listed below. Please note that some of this information may exist either exclusively or additionally in electronic or digital form, and this request must be construed to include any such information:

The current number of prisoners placed in administrative segregation, protective custody, or similar status, and who have been identified by DOC as (1) blind or as having a visual impairment; (2) deaf or as being hard of hearing; (3) blind and deaf, and/or (4) having a mobility impairment.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 5 business days, as the statute requires.
