Pursuant to the Ohio Open Records Law, I hereby request the following records:
All procedures and associated forms (blank) relevant to the reporting of inmate injuries and death at both public and privately-run correctional facilities, should differences apply.
I also request that, if appropriate, fees be waived as I believe this request is in the public interest. The requested documents will be made available to the general public free of charge as part of the public information service at MuckRock.com, processed by a representative of the news media/press and is made in the process of news gathering and not for commercial usage.
In the event that fees cannot be waived, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

A requester must identify the records he or she is seeking "with reasonable clarity," so that the public office can identify responsive records based on the manner in which it ordinarily maintains and accesses the public records it keeps. Hello Ms. Smith,
Thank you for asking for clarification. I would like blank versions of all forms that need to be submitted to the Department of Corrections when an inmate in custody dies or is injured, and I would like any policies/procedures that instruct the facility to disclose that information.
