Ms. Calkins -

The APRA request below has been faxed a few times to 401-233-1438 and failed. Could you please confirm receipt?

Thank you!

All records are 15 cents per page; therefore, I can contact you and let you
know how much it will be. If there is extensive time involved, the first
hour is free and then it is $15.00 per hour. The records are not released
until the fee is paid, and we will mail them to you, they cannot be sent
electronically or on a CD-ROM.

To Whom it May Concern:

I realize you were having difficulty our FAX machine. It had been
down for over a week but is now fully functional. Most requests are done
in person; however, I realize your company is located in Boston; therefore,
please send any future requests by FAX or mail. Please do not send
requests to this email, because no one else in the Records Section is able
to retrieve it, and if I am not in, the request cannot be filled.

Thanks you,

To Whom it May Concern:

I have checked the Dispatch Log from May 9, 2014 through May 16, 2014.
It is 189 Pages, which will be a cost of $28.35. These logs will have to
be redacted for any information that is not deemed public record. I do not
believe it would take more than an hour, so there should not be any
additional fee. Please let me know ASAP if you will be wanting these
records, and also a telephone number where we can reach you in the future
to notify you of the costs involved.

hi Shawn, I sent u an email before I left for.vacation asking you to not
send anything else to this email, to fax your request because I am the only
one who gets this email. I have been on vacation for over a week and will
not return until June 2nd, so again, please do not send any further emails
to this email address. Please send all requests either through the mail or
by fax to 401-233-1438 attention to Records. Thank you.

From: Shawn Musgrave

Ms. Calkins -

I sent you the email because I assumed it was the fastest way to confirm that I wanted you to continue processing the request. Is there another person's email that I might have to confirm whether to make the check to North Providence (city) or the police department directly?

I will make sure to send future requests by fax.

Thank you again for your help!

-Shawn

From: Lisa Calkins

Hi Shawn,

I was away on vacation for almost two weeks so there was no way of me
filling your request at the time. No one else has access to my e-mail

Section at 401-233-1438. Please leave a phone numbers so we may inform you
of the cost involved.

Thank you,

Dear Mr. Musgrave,

We received a fax from your company yesterday afternoon requesting the
records that you had previously e-mailed the department about. As was
stated the cost is $28.35 plus if it is over an hour to process the
documents, it will be $15.00 per hour. You did not give any telephone or
FAX number for us to contact you. Please furnish these numbers, not an
email address, for us to contact you in regard to the information you need,
in that anyone in the Records Section can process your request. We will in
turn call you with exact cost involved, and when the payment comes in, we
will then forward you the requested records. We are not set up for credit
card payments; therefore, you will have to send a check for the payment.
If you have any questions, please contact me, Lt. Anzivino at 401-231-4533,
ext. 112.
