Pursuant to the New York Freedom of Information Law, I hereby request the following records:
Computer file (i.e., spreadsheet file, database file) of the index of all Notices of Claim filed with the City of New York for personal injury claims for the years 2007 through 2017. If a computer file is not available, paper files may be produced.
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 5 business days, as the statute requires.

I received a response to my FOIL request for the index of Notice of Claim for personal injury filed with the City of New York for the years 2007 to 2017. The response is incomplete and a further response must be provided.
The Index provided by the City consists of an excel spreadsheet with complete calendar years for the years 2007-2016, and from Jan-June, 2017. The entire calendar year should be provided for 2017.
Moreover, Index provided in response to my request contains 4 data colums: Claim Number, Date of Filing, Claimant's Last Name, and Claimant's First Name. However, I know from having personally reviewed the indexes that additional information is maintained by the city. In addition to the 4 fields above, there are additional fields indicating the location of the occurrence. This information was not provided in the spreadsheet I received, but should have been.
Please provide a complete response to the FOIL request.

The agency’s appeals officer received your appeal on August 13, 2018. New York Public Officers Law § 89(5)(b)(2) provides that an agency shall issue a determination on an appeal “within ten business days of the receipt of the appeal.”
Accordingly, the response to your appeal is due by the close of business on August 27, 2018. In any event, as a courtesy, the responsive records are attached to this email and a formal written response will be sent to you on August 27, 2018.
