Any and all hiring and personnel records for Dr. Tom Frieden, for any and all paid, appointed, elected or volunteer positions he has held in your agency. Dr. Frieden was employed by your agency from approximately 2002 to 2009.
These records should include, but are not limited to:
• Job/Volunteer position applications
• Job titles, and, if available, job descriptions
• Resumes and CVs
• Letters concerning offers, hire, promotion, disciplinary proceedings and/or dismissals
• Documentation of commendations, recommendations and rewards
• Training records
• Contracts for services
• Salary and bonus records
• Disciplinary actions and/or investigations and/or decisions
• Letters of resignation
This information concerning his employment and volunteer history is provided to help you locate his records:
• 2003 to 2005 — New York City Department of Health and Mental Hygiene; Medical Director for Special Projects, Office of the Executive Deputy Commissioner
• 2006 to 2014 — New York State Office of Alcoholism and Substance Abuse Services; Medical Advisory Panel
• 20011 to 2012 — Office of the Governor; Inter-agency Work Group on Prescription Drug Abuse
• 2012 — City of New York, Office of the Mayor; Task Force on Prescription Drug Abuse
