Pursuant to the New York State Freedom of Information Law (1977 N.Y. Laws ch. 933), I hereby request the following records:

A copy of the police report generated by the NYPD pertaining to the death of Kiersten Cerveny on the night of October 3, 2015 (or the morning of October 4, 2015) in Manhattan. Cerveny’s death was widely reported by news media:
