New York State Public Health law(s) that requires COVID-19 immunization (starting December 2020 until now) as a condition of employment and/or maintaining an employment in either private and/or public setting such as(but not limited to): hospitals, nursing homes, doctors’ offices, schools, colleges, other institutions of higher learning, transportation, public government offices, police department, fire department, health department, sanitation department, any type of private business (i.e.. restaurant, grocery store, coffee shop) and/or any other business in public and/or private domain serving the public in New York City (NYC) and New York State (NYS).
2. New York State Public Health law(s) that requires COVID-19 immunization (starting December 2020 until now) as a condition of education and maintaining education in either/or private/public sector (pre-K through college)
3. New York State Public Health law(s) that requires COVID-19 immunization as a condition of attendance of any school/college (pre-K though college) related events such as but not limited to, graduation, prom, ice cream sales, theatre performances, afterschool clubs.

4. Detailed NYS/NYC DOH public hearing records and dates on issues/concerns related to lockdowns, COVID-19 vaccine, masking policies, COVID-19 testing mandates from September 2019 until now
5. List of providers of COVID-19 vaccine to NYS/NYC
6. Lists providers and locations of COVID-19 vaccine clinics, “pop up” tents on either private/public property (listed but not limited to entities under #1) supported/facilitated by NYS/NYC DOH
7. List of providers of Pfizer Comirnaty COVID-19 Vaccine to NYS/NYC
8. Copies of all Agreements/Contracts between Pfizer Comirnaty COVID-19 vaccine providers and suppliers to NYS/NYC
9. List of all providers of Moderna COVID-19 vaccine to NYS/ NYC
10. Copies of all agreements and contracts between all Moderna vaccine providers and NYS/NYC
11. List of all providers of Jensen and/or J&J COVID-19 Vaccine to NYS/NYC

18. Any reports on deaths as a result of COVID-19 vaccine made to NYS/NYC DOH (from December 2020 until now) made by private individuals, public/private entities listed under; but not limited to #1
19. Detailed records on COVID 19 vaccine logs kept at the NYS/NYC DOH facilities and institutions under NYS/NYC DOH surveillance/care: the vaccine logs should include (but not limited to) appropriate storage and handling when in care of the facility.

20. List of all suppliers of COVID-19 testing kits and at home testing kits to NYS/NYC
21. Any reports on the toxicity of the COVID-19 testing kits and at home testing kits, reported to NYS/NYC DOH by general public and/or other private and public entities such as (but not limited to) facilities and institutions listed under #1
22. COVID-19 testing kits Purchase Agreements and Contacts between all providers and NYS/NYC
23. List of all COVID-19 and Omicron preventive masks suppliers to NYC and Purchase Agreements between the suppliers, federal government, NYS and NYC

25. Copies of Agreements/Contracts between all COVID-19 testing providers/labs and NYS/NYC
26. Copies of Agreements/Contracts between all COVID-19 testing providers/labs and NYS/NYC
27. List of any and all files, records, papers or documents on all of the above listed issues that are required by Freedom of Information Act (FOIA), 5 U.S.C. § 552, The Sunshine Act 5 U.S.C. § 552b, New York Freedom of Information Law (FOIL). By law you are obligated to supply me with this information and make it available for public inspection, viewing and copying.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Please be sure to include any documentation regarding significant policy or decision making or significant events, or dealing with legal precedents or significant legal issues, which have a permanent retention period per the Retention and Disposition Schedule for New York Local Government Records (LGS-01). Additionally, please include documentation containing routine legal, fiscal or administrative information which have a 6-year retention period, along with documentation that has no fiscal, legal or administrative value which have not been deleted by the date this request is received.

As required by the Freedom of Information Law, the head or governing body of an agency, or whomever is designated to determine appeals, is required to respond within 10 business days of the receipt of an appeal. If the records are denied on appeal, please explain the reasons for the denial fully in writing as required by law.
