All closed workers compensation claims for the calendar 2017 for the Class Codes 7219, 8293, 8292. Specifically, I request a machine-readable file (ideally a CSV, XLS or related format) of tabular data that contains the following fields:
- Claim Unique identifier for each claim record
- Filing date of the claim
- Account Unique identifier to differentiate claims for each customer
- Business Type/Code
- Gender of injured worker
- County, City or ZIP where injury occured
- Date of Birth injured worker (or year)
- Hire Date on which the present company hired the injured worker
- Date on which the injury occurred
- Claim Description
- Claim Status
- Type of claim
- Body Part(s) injured
- Main cause of injury
- Describes the type of injury
- Total amount paid out in medical, indemnity and other miscellaneous payments
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Please accept the following in response to your October 27, 2018 Freedom of Information Law (FOIL) request. You amended your request on January 8, 2019, replacing the above NCCI class codes with the following NAICS codes: 4841, 48411, 484110, 48412, 484121, 484122, 4842, 48421, 484210, 48422, 484220, 48423, 484230. You also acknowledged, pursuant to our telephone conversation of December 18, 2018, that the Board tracks only awards (not claims paid out) and only have indemnity awards (no medical, no miscellaneous).
Attached please find a record that is responsive to your FOIL request as amended. Please be advised that of the following with respect to the attached: (1) there were four NAICS codes of those requested that had no claims for the date range given, namely 48411, 48412, 48422, and 48423, (2) for date fields, most were turned into month/year or just year rather than the exact date. The Board defined "claims for the calendar year 2017" as claims with a date of injury in 2017, and (3) a unique claim identifier was created to identify the individual workers' compensation case.
To the extent that you deem any portion of this response to your FOIL request to be a denial, it may be appealed pursuant to Public Officers Law (POL) § 89(4) to the Board's Executive Director at 328 State Street, Room 438, Schenectady, New York 12305.
